Have you had a Brow Tint, Henna or Hybrid Tint before? Required
Are you Pregnant? Required
Have you had an adverse reaction to hair colour? Required

I understand that the brow treatment I have requested involves the application of products that may cause an adverse reaction to my hair  skin or body.

I can confirm I've recieved no adverse reactions within 48hrs of the patch test. Required

We have a 24 hour cancellation policy in place. We require 24 hours notice to reschedule or cancel an appointment. If less than this is given 50% of the treatment total will be charged. In cases of a ‘no show’, 100% of the treatment total will be charged.
