Henna Brows Consent Form

Please use this form prior to your appointment, you will still receive a full consultation on the day. This will save time and prevent any unnecessary contact in the salon.

Have you had a brow tint or henna before?
Are you Pregnant?
Have you had an adverse reaction to hair colour?

I understand that the brow treatment I have requested involves the application of products that may cause an adverse reaction to my hair  skin or body.

I can confirm I've recieved no adverse reactions within 48hrs of the patch test.
